X-Force Archery LLC, DBA Ace Archers Liability Waiver

Please read the consent and liability release carefully before signing it.

In consideration of participation in activities at X-Force Archery LLC, DBA Ace Archers (“Ace Archers”), or having any equipment repaired by Ace Archers, I acknowledge and agree to the following:

1. I risk bodily injury, including paralysis, dismemberment, and death, as well as loss or damage to personal property.  Activities at Ace Archers involve the inherent risk of physical injuries or other damages during or after my participation in the physical activities. I acknowledge that the risks include, but are not limited to, injuries caused by improperly used equipment, over-exertion of a client, slip and fall, an unknown health problem, or even death. I assume all risk and responsibility involved in these physical activities.
2. I knowingly and freely assume all risk, and I, for myself, and on behalf of my heirs, assign, and next of kin, and release andagree to hold Ace Archers harmless, their officers, directors, facilitators, coaches, agents, and/or employees, and other participants, concerning  all injury, paralysis, dismemberment, and/or loss or damage to personal property from this date forward to the end of time, except the result of Ace Archers gross negligence or willful or wanton misconduct.
3. I will indemnify and hold harmless Ace Archers, including any officers, directors, managers, employees, agents, successors, and assigns, from any claim resulting from any injury, death, loss, or damage sustained or claimed by another person as a result of my negligence while at Ace Archers.
4. I allow Ace Archers, their officers, directors, facilitators, coaches, agents, and employees take photographs and video me to coach me and allow Ace Archers to use my likeness in advertising, print, internet, and television, without reference to my name or age.  I agree that the images may be combined with other images, videos, text, and graphics and cropped, changed, or changed. I agree that I have no rights to my images, and all rights belong to Ace Archers.
5. I understand that I will not bring valuables to Ace Archers, and Ace Archers will not be liable for disappearance, loss, theft, or damage to personal property.
6. Ace Archers reserves the right to terminate my involvement in any program at Ace Archers for any reason not prohibited by law.
